Introduction: Breast cancer (BC) is defined as multifocal when there are two or more malignant foci in the same quadrant and multicentric when multiple foci develop in different quadrants of the breast. Objective: Case report on the importance of contrast-enhanced mammography in the detection of multifocal/multicentric BC. Presentation of case: A 43-year-old patient who attended a specialized MC program in march 2022 with an external ultrasound that indicated a biopsy for a single and irregular nodule in the right breast (MD). Results: Prior to performing the biopsy, contrast-enhanced mammography (CESM) was performed, which in the recombined image showed at least 6 focal enhancements in two quadrants of the MD, one of which correlated with the reported nodule. In the reference ultrasound, the rest had no ultrasound representation. BIRADS-5 was classified. The findings were confirmed with the histopathology and radiopathology results in favor of multicentric carcinoma. Conclusion: In patients with suspected MC, a comprehensive preoperative evaluation is necessary with images that provide not only morphological but also functional information, as is the case with CESM.
